This is a multi-disciplinary team (Occupational Therapists, Physiotherapists, Nurses and Support Workers) providing short term intensive programmes of rehabilitation to either prevent hospital admission or facilitate discharge from hospital.
Who is this service for?
This service is for patients living in the South Gloucestershire area or who are registered with a South Gloucestershire GP, requiring short term intensive programmes of rehabilitation to either maintain independence, prevent hospital admission or facilitate discharge from hospital.
How does this service work / what's involved in this service?
The main aims of the service are:
- To prevent unnecessary admission to hospital by providing a Rapid Response Service
- To facilitate discharge from hospital
- To support people to regain independence through a planned programme of therapy

- Community nursing team
