Care and Support for Adults

Care and Support for Adults

Our service consists of Social Workers and Care Service Co-ordinators who undertake needs assessments and help you to plan the support you need, as well as supporting carers in their caring role.

Who is this service for?

This service is for any adult over the age of 18 who has care and support needs, and who lives in the Bath & North East Somerset area.

How does this service work / what's involved in this service?

Our team of Care Advisors, Social Workers and Care Service Co-ordinators will offer you an assessment and help you to plan the support you need, as well as supporting your carer if necessary.

Our aim is to promote your wellbeing and to help you live independently for as long as possible. We may refer you to our Reablement Service to help you maintain your independence, or to a domiciliary care service as part of your care and support plan. Carers are also entitled to their own assessment.
