After a stay in hospital and once you are medically fit, returning to your own home is good for your health and wellbeing.

Our community teams will help you to return home as quickly as possible.

They will also help with your continued rehabilitation if you need it.

Planning to go home from hospital

When you are medically well, we will work with you and your family/carers so you can return home from hospital as soon as you are safe to do so.

We will discuss this with everyone involved prior to your discharge date, and will include all appropriate community services to make sure you are well supported once you leave hospital.

When you get home

On the day you go home you will be visited by our Community Physiotherapists or Occupational Therapists.

We will assess you to find out what community or social care services you need.

We will work with you to help you to remain independent and to enable you to stay at home.

We will also help you to find other services and resources to support you to manage your ongoing needs and to ensure your safety at home.
