Who is this service for?
Anyone over the age of 18 who is registered with a Bristol GP can access this service. It is appropriate for anyone with a neurological impairment such as Multiple Sclerosis, Motor Neuron Disease, Parkinson’s, Stroke, Spinal Cord Injury and many more.
What we do
The Bristol Specialist Community Neurology Service is led by a range of healthcare professionals who support patients with a neurological impairment.
Our service offers an assessment and discussion of your current needs, and will work with you to provide goal-led treatment using shared decision making principles. Our specially trained occupational therapists, physiotherapists, speech and language therapists and therapy assistants can work with you on any aspect of your health and daily activities that are most important to you.
We can see you in your home or at another address, and will make a thorough assessment to ensure that we can advise appropriate treatment, make suitable adaptations and offer equipment that may help. We will offer support and advice for as long as it takes to reach agreed achievable targets, and we will be available beyond this point should you need to get in touch.
How do I access this service?
As an individual you can refer yourself to this service, or you can ask a family member or carer to complete the form for you. If you are completing the form on another person’s behalf, you must ensure you have their consent to do so.
Referrals can be sent by fax, post or email, by completing the referral form with as much detail as possible.
