Who is this service for?

We work with people who are registered with a doctor in South Gloucestershire. We work with people from the age of 18.

What we do

The community learning difficulties team is made up of different professionals who work with adults who have a learning difficulty. We work with people to support their health needs.

We work in community settings, for example: at home, college, day service, work places or doctors surgery. We also have hospital liaison nurses that work in Southmead Hospital.

We work with people to help them to understand and make choices about their health. Sometimes we will work directly with the person who needs support. We also work indirectly through family or support workers. We will help set goals and make a plan to help meet needs.

  • Positive Behaviour Support
