Who is this service for?
The service is for adults (aged 18 years or over) with Type 1 or Type 2 diabetes who need the clinical expertise of a Diabetes Specialist Nurse. Anyone who is registered with a GP in the South Gloucestershire area can be referred to the service.
What we do
We provide service users with a comprehensive, specialist assessment of their diabetes, including any necessary treatment changes. Individual management plans and diabetes targets are developed to help people to get better control of their diabetes and to reduce their risks of developing diabetes complications which can affect their quality of life.
Examples of the types of referral to the service are:
- to initiate insulin or other injectable therapies
- insulin management skills – for people who need to learn how to adjust their own insulin safely and correctly
- treatment changes – for people who need a diabetes medication review in order to improve their blood glucose levels
- hypoglycaemia management – for people who are having problems with low blood glucose levels (hypoglycaemia) and need help to prevent/treat correctly
The service also offers sessions to people with Type 1 or Type 2 diabetes so they can learn how to manage their condition (this is also known as structured education and is usually delivered to small groups).
How do I access this service?
This service is available in a number of community venues across South Gloucestershire. You can be referred by your GP or another health professional.
