Who is this service for?

This Community Therapy Teams and Beds service is for adults over the age of 18 who are registered with a Bristol GP.

The Community Therapy Teams service is mainly accessed by people who require assistance in managing the effect of their long term conditions, but can extend to any adult who would be unable to attend an outpatient appointment for health reasons. For example:

  • People who are having difficulty walking
  • People who are having difficulty completing their activities of daily living (such as washing and dressing).

The Community Therapy service is split into the Community Therapy Teams, and the Community Therapy Rehabilitation Beds team.

Community Therapy Teams

The Community Therapy Team is made up of occupational therapists, physiotherapists, therapy assistants, community mental health nurses and speech and language therapists. The team provides occupational therapy, physiotherapy and speech and language therapy at a person’s place of residence.

There are three locality based community therapy teams, each providing the same service. There is one team based in North Bristol, one in East Bristol and one in South Bristol.

Community Therapy Beds

The Community Therapy Beds Team support health care and rehabilitation in allocated beds in residential and nursing units across the city. Depending on the unit, the team provides Physiotherapy, Occupational Therapy, Nursing and Pharmacy support.

The team works collaboratively with the individual, carers, local GP and unit staff to provide a package of rehabilitation with the aim of enabling the person to return to their own place of residence.

What we do

We enable people to rehabilitate and find self-management strategies in order to overcome the physical, psychological and social problems caused by ill health.

We do this by helping individuals (and their carers) to adapt their techniques, carry out exercise programmes, use adaptive equipment and learn more about their condition.

We also offer:

  • Rehabilitation programmes for people who have been in hospital, or have had a recent illness and suffered deterioration in their walking or function.
  • Advice on management of long term conditions such as arthritis
  • Advice to carers on subjects such as moving and handling.
  • Advice on pressure care involving seating or postural management.
