Assistant Practitioner -SIR572

The Frailty Service is a multi-disciplinary team who are seeking to appoint an Assistant Practitioner to join the team.

We provide a service to frail patients with many physical and mental health issues living in their homes or living in Nursing/Residential Nursing Care. Referrals are made via the GP, the frailty team complete a Comprehensive Geriatric Assessment with the patient/families in their homes helping them to set goals to improve their lives. The Assistant Practitioner role is varied, interesting and provides an opportunity to work autonomously as well as with the team where you will receive training, supervision and support.

You will need a range of clinical skills as well as excellent verbal and written communication skills, able to successfully use IT, organise and plan your diary through EMIS, liaise with a multitude of professionals and have a creative approach to supporting patients. A car driver is essential to this post as you will be travelling across South Gloucestershire.

Charlton House is a modern Community Resource Centre (CRC) based in Keynsham, Bristol. Charlton House has become a home for people who need both nursing care and those who do not need a nurse but do need a higher level of staffing. This is a friendly homely care establishment with a caring team.

We have a number of casual shifts available which includes the early shifts (starting at 8am - 2pm or 3pm) and late shifts (2pm or 3pm ending at 9pm or 10pm). We also have shifts available on weekends which enhancements are paid.

The successful applicants will support the residents with some or all aspects of daily living, including personal care, activities and maintaining contact with family and friends when required. You will help with planning and organising activities for residents, helping residents to maintain personal hygiene and assist residents and their families in the provision of care during illness and at end of life.

Rates of Pay for Support Workers on a casual basis:

Day Shifts: £9.83 per hour

Behaviour Nurse – SIR577

South Gloucestershire Learning Difficulties Service are seeking a Behaviour Nurse to work within our Positive Behavioural Support Pathway.

The Behaviour Nurse role is to support the Positive Behaviour Support Team to deliver complex assessment and intervention to support service users who display behaviours which may challenge services and/or have mental health needs.

The team consists of members from specialities within the Community Learning Disability Team who have skills in Positive Behavioural Support including Occupational Therapy, Speech and Language Therapy, Behaviour Nursing, Psychology and Psychiatry.

The role will involve completing functional assessments, formulations and plan interventions both individually and in liaison with other team members. The Behaviour Nurse also has a key role in allocating referrals, ensuring the running of team functions as well as providing leadership and management to junior staff.

The role will be based within the South Gloucestershire Learning Difficulties Service at Church House, Soundwell and involves regular community visits to family homes, residential and supported living services, day centres and other health and social care establishments. Typical hours are Monday to Friday 9am -5pm although there may be occasional changes to this to meet service needs.

Care Support Worker – SIR575

Sirona care and health are establishing a new care and support service to work alongside our community rehabilitation teams in both Kingswood and Severnvale localities in South Gloucestershire. This new team will be delivering home care and reablement in people’s homes. You will be part of a long established team, supporting people who have been discharged from hospital and or following rehabilitation who required long term care. Support to undertake the Care Certificate will be available if you do not already hold this, along with access to other development opportunities.

Supervision and support will be available from the therapy rehabilitation teams. You will be required to cover morning and evening 6 hour shifts from 8am-2pm and/or 4pm-10pm and will need to be available to work weekends on a rota basis..

Successful candidates will hold a full driving license and need access to a car. You will be paid mileage and be provided with a mobile phone, a tablet and a uniform and have access to our corporate induction and on-going training. We are looking for a practitioner to ensure high standards of clinical care are maintained at all times by providing strong, effective professional and clinical leadership.

The focus of the work will be working autonomously within a range of outpatient settings offering highly Specialist Assessment, treatment which is evidence based, collaboratively with young people and their families. Working collaboratively with GP’s advising of specialist care and treatment plans for children in adherence with NMC guidance and in conjunction with the child or young person’s Community Paediatrician where appropriate.

The post holder will support the on the ongoing review and further development of existing care pathways, policies and audit of service outcomes, develop and review waiting list initiatives, working collaboratively with commissioners and referrers achieving agreed targets. The post holder will have excellent communication skills working across multi-disciplinary teams.Delivering training and supervision to other professionals.

Provide a range of appropriate, evidence based clinical interventions informed by assessed individual need. The post holder will be working with children and young people with serious and complex mental health needs often in complex family situations displaying a range of risk type behaviors. We have an exciting opportunity for a registered nurse to join our out of hours service to cover up to four twilight shifts per week, working 4.30pm - 11pm across South Gloucestershire.

The post holder will:
-Assess, plan, evaluate and deliver individualised patient care.
-Undertake terminal care inclusive of once only injections/setting up syringe drivers for symptom management/pain and changing doses of drugs, including controlled drugs, within the parameters as prescribed by GP according to the patient’s needs.
-To have knowledge of the drugs used in symptom control management.
-Have experience and knowledge of IV-therapy and extended catheter management.
-Complete assessments of clients using observation, measurement, verbal and written documentation, including medical history, and environmental factors, without direct supervision; including complex care assessments in consultation with senior grade.
-Advise and recommend treatment, liaise with other Health Care Professionals accepting referrals written and verbal.
-Participate in clinical audits and data collection.

You will be compensated for any work travel at 45p per mile.

Rotational Physiotherapist -SIR581

Are you looking for a new challenge to deliver care in a community setting with the opportunity to experience a broad range of clinical experience through a rotational post? We are looking for an enthusiastic, motivated an skilled clinician to contribute to the physiotherapy rotation at junior level.

Each rotation is six months in duration and opportunities are available in; Community Respiratory Specialist team, Community Rehabilitation team (includes rapid response), Elderly Care, Falls team, Adult Learning Disabilities (includes hydrotherapy) and Musculoskeletal out-patients.
The Community Respiratory Specialist Team are based at Leap Valley Medical Centre and this rotation will help support patients with self-management of their respiratory conditions to include the prevention of hospital admission, early supported discharge, home oxygen assessment and review and pulmonary rehabilitation.

The Falls service is based at Downend Clinic with clinics and groups at Cossham hospital, Thornbury, Patchway, and Yate. There will also be the opportunity to meet patients in their home environment to prescribe rehabilitation by ensuring a holistic approach by liaising with the rest of the Multi-disciplinary team (MDT).

The musculoskeletal outpatient team is set in different locations within South Gloucestershire and provides assessment and treatment of adults with a variety of musculoskeletal conditions; these may be acute problems such as post-fracture or surgery but also chronic conditions such as low back pain. We also run a number of education and rehabilitation classes in South Gloucestershire.

Elderly care multidisciplinary team in acute elderly care admissions and rehabilitation with a bias towards neurology conditions. This is a Rehabilitation, re-ablement and recovery unit and it supports people back to optimal function and independence, with the goal to return people to their homes, or if this is not possible to a more appropriate location.

The community rehabilitation teams provide rehabilitation to adults within their own homes to prevent hospital admission and to facilitate hospital discharge. This rotation will be in settings across South Gloucestershire and is a flexible service, 7 days per week including bank holidays. This rotation gives the opportunity to work as part of a multidisciplinary team in a number of specialties.
The team working with Adults with learning disabilities provides an opportunity to learn novel ways of communicating, and help some of the most vulnerable people in our community to manage complex physical and learning disabilities. This rotation gives the opportunity to provide rehabilitation in a hydrotherapy pool.

We are looking for qualified physiotherapists to provide physiotherapy assessments and treatments across the above settings on a rotational basis. The successful candidate will have supervision and mentorship from senior physiotherapists in the local team and will also be offered training relevant to the specialty.

Not all the bases are accessible by public transport therefore the post holder must hold a valid UK driving license and have access to a car which is insured for business use.

Requirements of the job:

This role is for a fixed period of 9 months.

The role will require the post-holder to provide overall transformation management and development for Adult & Specialist Services including Learning Disability Services in South Gloucestershire, Bristol, reporting to the Head of Adult & Specialist Services. You will provide project management expertise for a number of high profile projects, which will involve the production of plans, documentation and the provision of guidance for project teams and ensuring communication is maintained between all stakeholders throughout the project lifecycle, helping to ensure all organisational procedures are adhered to, objectives are identified and met, and benefits realised from the implementation of the change.

Integral to the role is representing Sirona by working in partnership with other BNSSG providers. There is a requirement to attend a significant number of system wide meetings and the occasional requirement to deputise for the Head of Adult & Specialist Services and the Director of Operations.

You will need to demonstrate sound knowledge and experience of working in the area of health or social care and have experience of supporting front line service delivery in accordance with the Health and Social Care Act (2008) and the requirements of CQC regulations.
