Assistant Practitioner

Exciting opportunities have arisen for a dynamic, enthusiastic Assistant Nurse Practitioner interested in working in the community within the Kingswood Localities.

The Teams are friendly, supportive and hardworking and have a real commitment to delivering excellent patient centred care as part of the wider multidisplinary team.

We are looking for nurses with excellent decision making, organisational, prioritsation and time managment skills who are able to work alone, and as part of a team and support the team leader to deliver compassionate, dignified care. You will also need excellent communication skills including negotiating, diplomacy and an ability to deal with sensitive issues. You will work closely with the team leader and the team and need to have a good understanding and ability to deliver many clinical skills (palliative care, wound care, safeguarding, complex patients, and prevention of unnecessary admissions) and complex situations. You will need to be able to form excellent working relationships with all levels of staff including GP Practices and hospital services.

Catering Assistant

As one of our important Catering Assistants at Combe Lea Community Resource Centre, you will work as part of a small team delivering a service to the residents at Combe Lea, Pinelea Day Centre and the Extra Care Service.

Senior Inpatient Physiotherapist

The South Gloucestershire Community Rehabilitation In-reach Team provides in-reach therapy to people who are being supported in the short term Discharge to Assess Pathway 2 beds in SG. These beds are hosted in a Community Rehabilitation Unit on the Thornbury Hospital site and also in Nursing and Residential Homes throughout SG. They provide an environment committed to encouraging active participation in and engagement with dynamic rehabilitation, re-ablement and recovery for people who are not safe to be discharged directly home following an acute stay but have on-going needs to be addressed prior to being discharged to their place of residence. The team work within local MDTs and in close partnership with colleagues in the nursing facilities, Social Care and other Community services.
