The South Gloucestershire Community Rehabilitation Team (CRT) is an innovative and dynamic service. We provide active rehabilitation to patients in their place of residence to maximise their functional potential and support them to remain independent within their homes. We aim to assist in preventing hospital admission and to facilitate timely hospital discharge if an admission to the acute trust has been unavoidable (Discharge to Assess). We also provide Planned (Domiciliary) therapy to ensure that patients are assessed and supported early in the journey to maintain their optimal level of independence and hence avoid longer term concerns.

We are seeking an enthusiastic and motivated physiotherapist to join our busy and dynamic CRT. We are looking for a physiotherapist with some experience (preferably of community working) to promote independence and choice for our service users and their carers.

The post is full time and you would be based at Patchway Clinic working across the Severnvale Locality.

In return, we will give you the opportunity to develop specialist community skills and enhance your multidisciplinary working; we will provide you with regular training and supervision opportunities as well as the opportunity to develop your leadership skills in supervising Rehab Support Workers.

You will be working within a friendly and supportive multidisciplinary team alongside other Physiotherapists, Occupational Therapists, Speech and Language Therapists, Rehabilitation Support Workers and Physiotherapy Technicians. We collaborate closely with our Community Nursing colleagues and other Acute, Community and Social Care partners. We offer a flexible service which covers extended hours operating 7 days a week, to support our service user needs.

Working arrangements include weekends and bank holidays on a rota basis.

Cleeve Court is classified as a Dementia Care Matters Butterfly Service® home which has achieved Level 2 on the Qualitative Observational audit. This audit was conducted in June 2017 and described the Cleeve Court ‘Level 2 (out of a possible 9) – A highly skilled, loving care with a high degree of person-centred dementia care and potential to develop into a Level 1 home’.

The CQC inspection of September 2016 rated Cleeve Court as ‘Good’. To help Cleeve Court reach its next goal of becoming a ‘Level 1 home’ and achieve an ‘Excellent’ CQC rating, we are looking to recruit a team of motivated casual (bank) Support Workers!
