We have an exciting opportunity for a Community Nurse to join our out of hours service for 16 hours covering twilights & nights; twilights work 4.30pm – 11pm and nights are – 22:30pm – 8am across South Gloucestershire but based at Cossham Hospital
The post holder will:
Assess, plan, evaluate and deliver individualised patient care.
Undertake terminal care inclusive of once only injections/setting up syringe drivers for symptom management/pain and changing doses of drugs, including controlled drugs, within the parameters as prescribed by GP according to the patient’s needs.
To have knowledge of the drugs used in symptom control management.
Have experience and knowledge of IV-therapy and extended catheter management.
Complete assessments of clients using observation, measurement, verbal and written documentation, including medical history, and environmental factors, without direct supervision; including complex care assessments in consultation with senior grade.
Advise and recommend treatment, liaise with other Health Care Professionals accepting referrals written and verbal.
Participate in clinical audits and data collection.
Contribute to service development and link nurse role.
Manage the daily Patient Caseload and capacity during your shift within agreed boundaries.
