Working with Partners

Working together for the benefit of our community

It is important to us that we work with individuals and partner organisations to ensure the care you receive is as joined-up as it can be.

This can be by service with our staff joining multi-disciplinary teams so that everyone involved in your care knows what you need and when; it can also be by our involvement in the wider health and care system. The video above describes how multi-disciplinary teams work.

We also provide some services with partners. The Community Children’s Health Service in Bristol and South Gloucestershire is run by us working closely with Bristol Community Health, Avon and Wiltshire NHS Partnership Trust (AWP),  University Hospitals Bristol NHS Foundation Trust and Barnardo’s.

As an organisation, we are involved in the STP – Sustainability and Transformation Plan – in Bristol, North Somerset and South Gloucestershire.

NHS England set out a new approach to help ensure that health and care services are built around the needs of local populations.

To achieve this all those involved with NHS and Local Authority funded care have been asked to work together to create a local plan for the next five years and this is called an STP.
