The Sirona Community Rehabilitation Team (CRT) provides active rehabilitation to patients in their place of residence to maximise their functional potential and support them to remain independent within their homes. We aim to assist in preventing hospital admission and to facilitate timely hospital discharge if an admission to the acute trust has been unavoidable. We also provide Planned (Domiciliary) therapy to ensure that patients are assessed and supported early in the journey to maintain their optimal level of independence and hence avoid longer term concerns. The CRT is a multidisciplinary team that provides a flexible service, working extended hours and 7 days a week, to support our service user needs.
Are you ready to take on a new challenge and join a team that is supporting the residents of South Glos to make meaningful improvements in their lives and enhance their physical and emotional well-being? Do you have a passion to deliver exceptional Community-based therapy and support our service users to improve their quality of life and optimise their function? If the answer is Yes, you could be the person we are looking for.
We are seeking an enthusiastic and innovative physiotherpist to join our busy and dynamic CRT in South Gloucestershire. We are looking for a physiotherapist with some experience (preferably community) to work in a Community setting to promote independence and choice for our service users and their carers. The post is full time and you would be based at Downend Clinic working accross the Kingswood Locality.
Successful candidates will support the delivery of new initiatives in SG, working in collaboration with our Acute and Social Care partners, including Planned Therapy Service and Discharge to Assess model, supporting people to return to their own homes to undertake ongoing assessments and recovery.
You will be working within the Locality CRT that includes Physiotherapists, Occupational Therapists, Speech and Language Therapists, Rehabilitation Support Workers and Physiotherapy Technicians and will collaborate closely with our Community Nursing colleagues and other Acute, Community and Social Care partners.
