Who is this service for?

This service is for people with a learning disability and their carers, living in Bristol,  North Somerset and South Gloucestershire who are registered with a GP in the area.

What we do

The community learning difficulties team is made up of different professionals who work with adults who have a learning difficulty. We work with people to support their health needs.

We work in community settings, for example: at home, college, day service, work places or doctors surgery. We also have hospital liaison nurses that work in Southmead Hospital.

We work with people to help them to understand and make choices about their health. Sometimes we will work directly with the person who needs support. We also work indirectly through family or support workers. We will help set goals and make a plan to help meet needs.

  • Physiotherapy/Hydrotherapy
  • Occupational Therapy
  • Speech and Language Therapy
  • Psychological Therapies
  • Psychiatry
  • Community Nursing
  • Hospital Liaison Nursing
  • Positive Behaviour Support

How do I access this service?

To access this service you need a GP Practice within the Bristol, North Somerset and South Gloucestershire area.

We have a new referral form which will be used across the area. You can download the form using the link below and send your completed form to Sirona.CLDTref@nhs.net
