Support for Older and Disabled People

Sirona Care and Health works with Bath and North East Somerset Council to provide a range of services which help older and disabled people to remain independent for as long as possible and to exercise maximum choice and control over their lives.

How do I get this service?

Anyone who is over 18 years old and seems to be in need of services can ask for a community care assessment.  A community care assessment is a method of working out what your needs are and, where possible, matching these to services which will help you to live the life you choose.  The assessment will normally be done by a social worker or another professional employed by Sirona Care and Health.

The worker will usually come to see you at home to ask you some questions and discuss your situation.  He or she will then discuss various options with you.  Usually this will involve more than one visit.

If you are assessed as being eligible for services, these will be organised in the form of a Personal Budget.

What is a Personal Budget?

Personal budgets are the new way of delivering social care services.  They are a way of helping people to take control of their own lives by managing their own support services.

Personal budgets allow you to have greater control over the way your support and care are organised and which organisations or individuals provide the support.

There are three ways of organising and paying for your assistance and support within a personal Budget:

1.  Support organised by Sirona Care and Health

2.  A Direct Payment

3.  Using an Agent / ’Managed’ Personal Budget

Will I have to Pay for My Services?

All social care services are means tested so you may have to pay a contribution towards the cost of any services.  For more information about ‘Fairer Charging’ click here.
