What is the Active Ageing Service?
The Active Ageing Service works in partnership with your GP surgery. We aim to offer an annual home visit to everyone between the ages of 80-84 years; and those who are 85+ may be visited by specific referral.
The service is provided by Health Visitors who are Specialist Community Public Health Nurses, together with experienced Health Visitor Support Workers. The Active Ageing Service aims to enable older people to continue to be independent and to promote quality of life in their own homes.
We would like to work in partnership with you to identify any health or social needs, and we can also direct you to services which may be helpful to you.
The Service offers:
- Advice on preventing falls and accidents
- Advice and support with memory loss or dementia
- Support for those who have been recently bereaved
- Advice on living well with on-going health conditions
- Health promotion
- Advice and support for carers
- Promoting quality of life for older people within their homes and communities.
- Signposting to local and national support services
We work in partnership with your GP surgery and other health and social care professionals.
We welcome individuals contacting us after home visits when needs or situations change. If you are over 85 years old and would like to meet us, please ask your GP for a referral.

Sirona care & health is a Community Interest Company (CIC) that is committed to providing health and social care services to people within their own communities.
Sirona care & health is a non-profit organisation which is funded by NHS and public sector monies. There is no charge to you for the Active Ageing Service.
Sirona care & health has provided services in South Gloucestershire since 2014.
We are accountable to NHS bodies for the standards of service we provide and are inspected regularly by the Care Quality Commission.
With your consent information may also be shared with other professionals involved in your care. Your information may be used anonymously by the National Health Service (NHS) to provide data to develop future services.
